A job application form is used by employers to obtain information about prospective employees. For a modern business, this form is available electronically as well as printable format. The form is often created in an Adobe Acrobat PDF format. The PDF job application form can be created with electronic form fields to make it easy for job applicants to fill out on a computer .

The job application form has been used by employers for decades. For many job positions, it is the only document used to assess whether a job seeker is qualified for the job position.

It is important to ask the right questions in the job application. Of course, you need to have an area for name, address and contact information. However, you also need to find out when the employee is available for work and their salary requirements.

There should be a section on education where the job seeker can fill out information about high school or college education, years attended and degrees. Employment history should obtain information about the company, dates employed, city, job title and company contact information. Many forms do not ask starting and ending salary but this important information to find out if this job seeker advanced. You also want to see if their most recent job salary is close to their salary requirements. Lastly, you should ask their reason for leaving the company while acquiring the name of their supervisor with a place to check or ask if you can contact their previous supervisor.

It is a good idea to include a section to document licenses and certifications in the job application form. Make sure to have a space for certification or license numbers and the state it was received. This could be used for licenses professionals or possibly a truck driver with special drivers license.

You need to also include an area to list references. This way if the job applicant looks good, you will have these references available to call and find out more about the application.

The last page of the job application form should be the formal area where you include any statement of certification, signature and date.
